Overview

Title:            Physical Therapist Occupational Health

Location:     MT. Vernon, IL

Status:        Full Time

Hours:         M-F Day shift

 

 

Early-career Physical Therapist. Focus in this role is understanding and integrating ATI’s Care Model into standard practice, leveraging available tools and resources to guide and optimize care plans and patient outcomes/experience.

Responsibilities

• Performs evaluations and establishes a goal-based plan of care
• Implements regular objective reassessment points to track progress towards goal achievement and/or adjusts goals based on patients’ recovery trajectory
• Establishes care plans aligned to ATI Best Practice guidelines, to achieve predictable and positive outcomes
• Manages his/her Caseload effectively, across the spectrum of each patient’s plan of care
• Effectively delegates treatment among the clinical care team
• Meets or exceeds established performance expectations, including patient outcomes collection targets, scheduling practices, productivity, caseloads management, and documentation compliance
• Communicates effectively and regularly with referral sources and other relevant stakeholders in a patient’s plan of care (e.g., physician, family members/ caregivers, case manager, attorney, etc)
• Assists with daily clinic operational needs and tasks as needed

Qualifications

Minimum Education
Required:
• Minimum Bachelors of Physical Therapy from an accredited institution
 
 
Minimum Experience
Required:
• No prior experience required as a licensed PT if graduation requirement is met

• Designs and structures treatment plans to enable best possible patient outcomes, leveraging tools, resources and care team personnel effectively
• Delegates care amongst the care-team effectively based on case complexity classification to enable all members of the care team to practice at the top of their license
• Ability to effectively manage a care episode across a caseload of patients
• Ability to set objective and achievable goals
• Ability to adjust treatment plans based on patient outcomes data and measurable progress
• Demonstrates best practices with patient schedule management
• Strong communication skills, written and verbal
• Team player across the clinic and district
• Flexible and adaptable to appropriately service patient needs and contribute to positive experience and outcomes
• Completes work tasks in a timely and accurate manner
• Operates with integrity – ensures work contributes positively to ATI’s standards and practices, all within Compliance guidelines
 
Licenses and Certifications
Required:
• Licensed PT in-state, or candidate for licensure in-state
