Overview

The Virtual Care Navigator Manager is responsible for leading the team of clinicians, Virtual Care Navigators, who support patients through our Remote Therapeutic Monitoring (RTM) service line. With a focus on ATI’s mission, vision and values the Virtual Care Navigator Manager will develop a high performing team which consistently delivers growth, operational performance, and an extraordinary customer experience. The Virtual Care Navigator Manager interacts with and supports a level of involvement with both the Virtual Practice and the Virtual Operations teams for the purposes of optimizing the Remote Therapeutic Monitoring service line, workflows and customer experience at ATI.

Responsibilities

Management of Virtual Care Navigator Clinical Team
• Effectively support and oversee the team of clinicians who provide Remote Therapeutic Monitoring (RTM) services for all patients at ATI
• Build a team culture which aligns with the company’s mission and values and support development of clinical team members to drive high performance
• Communicate key messages in a timely manner to the team, acting as liaison between corporate communications and clinical team
• Manage the full performance review cycle for direct reports, including performance feedback discussions, performance documentation and delivering performance improvement plans
• Manage daily operations including, but not limited to, payroll processes and employee scheduling
• Manage time off approval, requiring coordination and coverage alignment across multiple states based on licensure

 

Patient Monitoring and Support
• Responsible for supporting a caseload of Remote Therapeutic Monitoring (RTM) patients in addition to driving performance of the Virtual Care Navigator clinical team, each with their own caseload of RTM patients
o Continuously monitor patient data and treatment adherence through remote monitoring systems to optimize patient care, experience and outcomes
o Track and document patient progress, symptoms, and treatment side effects
o Maintain regular communication with patients to provide support, answer questions, and address concerns
o Facilitate virtual check-ins to ensure patients understand their treatment plans and adhere to prescribed therapies
o Effectively leverage Remote Therapeutic Monitoring (RTM) clinical dashboards to prioritize and optimize patient care while also achieving RTM billable standards
o Ensure accurate and timely documentation for all patient interventions and bill effectively for Remote Therapeutic Monitoring (RTM) services rendered
o Consistent communication and coordination with other clinician team members across ATI to optimize treatment outcomes and patient experience
• Management of patient caseloads related to Remote Therapeutic Monitoring (RTM) to optimize support and care provided to each patient by the Virtual Care Navigator clinical team throughout the therapy journey
• Timely documentation review and documentation sign-off for Virtual Care Navigator clinical team

 

Coordination with Healthcare Providers
• Work effectively as an integral part of a multi-disciplinary care team with both In-Clinic and Virtual Care Clinic Directors and District Directors
• Proactively communicate with and work with the Virtual Practice team and the Virtual Operations team to problem solve, identify solutions and continue to optimize the Remote Therapeutic Monitoring (RTM) service line
• Act as an advocate for patients, addressing any concerns or barriers they may encounter in accessing virtual care services and adhering to treatment
• Coordinate adjustments to treatment plans based on patient feedback and monitoring data by relaying important patient information to the primary clinician

 

Technical Assistance and Patient Education
• Assist patients in setting up and using remote monitoring devices and applications, ensuring they are comfortable and confident in using the technology
• Troubleshoot technical issues and provide guidance on using health management software
• Educate patients and their families about their conditions, treatment plans, and the use of remote monitoring tools
• Provide resources and materials to support patient self-management and understanding of their health conditions

Data Management and Reporting
• Prepare regular reports for Virtual Practice and Virtual Operations management that reflect metrics specific to Remote Therapeutic Monitoring (RTM) performance
• Based on Remote Therapeutic Monitoring (RTM) data, identify and address workflow, utilization and operational opportunities to optimize the service line

Qualifications

Minimum Education
Required:
• Doctorate or Master’s degree – Physical Therapy

 

Minimum Experience
Required:
• 3+ of experience as a licensed PT
Preferred:
• 2+ years of leadership experience
• 1 or more years of experience in Telerehab
• 1 or more years of experience in Outpatient Physical Therapy

 

Knowledge Skills and Abilities
Technical Skills
• Proficient in using remote patient monitoring systems and electronic health record (EHR) systems
• Strong computer skills, including familiarity with telehealth platforms and health management software
• Ability to utilize and leverage reporting tools to assess and optimize performance metrics

• Excellent communication and interpersonal skills, with the ability to convey information clearly and empathetically
• Strong organizational skills and attention to detail
• Ability to work independently and collaboratively in a remote environment

 

Professional Skills
• Experience in leadership with ability to demonstrate initiative, critical thinking, adaptability, dependability, mentorship and team-building skills
• Strong problem-solving abilities and a proactive approach to identifying solutions
• Knowledge of healthcare policies, remote monitoring regulations, and patient privacy laws (e.g., HIPAA).

 

Licenses and Certifications
Required:
• Licensed Physical Therapist in home state and eligible for licensure in multiple states as needed, as defined by ATI
Preferred:
• Licensure in more than one state in which ATI operates
